AI Incident Database (AIID)

Updated 05/18/26The AI Incident Database (AIID) is a centralized, systematized repository of incidents where artificial intelligence systems have caused or nearly caused harm. Modelled on incident databases in aviation and computer security, it records and organizes these events so that stakeholders can study patterns in AI failures and use that knowledge to prevent or mitigate similar bad outcomes in the future.
Theory of Change
By systematically collecting and indexing incidents where AI systems have caused or nearly caused harm, the AI Incident Database creates a shared evidence base of AI failures. This record enables practitioners, researchers, and policymakers to analyze how and why harms occur, inform risk management practices and standards, and ultimately prevent or mitigate similar bad outcomes when AI systems are deployed in the future.
